This
form gathers all the information necessary for creating a performance
appraisal for a specific person or a review of some other specific
target (e.g. school).
If
you are setting up a performance appraisal for a specific person,
there is a hardcopy of this form available for download by clicking
here. You can print
this form and use it to collect the necessary detailed appraisal information
during the preparation meeting/process. If you are setting up an online
survey for something else other than a role appraisal (e.g. review
of a school), there is a version of the hardcopy form specifically

Before
completing the form below it is important to do the following:
1. Identify,
revise or develop the question bank that you want to
use for this role appraisal or review of something else (e.g. school).
You can review the question bank development process by clicking here.
If you have already developed a question bank for this type of appraisal
or online survey, you can check that it has been set up in the question
bank database by looking for the name of the question bank in the
drop down list below (see please select the question bank…).
2. Identify
the various response groups that you want to target
(total of 8 response groups are available including the person being
appraised if the process is a 360 degree performance appraisal process).
The response groups should have been identified during the preparation
process. Please remember that you if you have more than 8 response
groups, you need to think about how best to merge some response groups
in order to produce the best possible final report).
3. Develop
the mailing lists for each of the identified response
groups (e-mail, physical addresses for both). Click here

4. Decide
whether your organisation, Ardjuna or both parties are
going to manage the process of mailing out the invitation
to participate letters and reminders to members of the various response
groups.Please note that Ardjuna do not handle mail-out of hardcopy surveys within the cost structure of the 360 degree online process. If the Project Manager is going to manage the

5. Secure
any customized questions that will be added to the question
bank for this particular appraisal/online survey
